Forest Care Selection are currently working with a unique and well established homecare provider.

This Outstanding rated provider are seeking a Care Coordinator to work with their Clinical Nurse Manager in the delivery of the very highest standard of perse centred care to a variety of individuals in their own homes.

The Care Coordinator in Chelsea will:
• Provide a safe, caring, effective, responsible, and well-led service of care to the organisation’s clients.
• Ensure continuous improvement and assist in the maintenance of this provider Outstanding CQC rating.
• Ensure outstanding quality standards of care by working effectively with the care and nursing team in the field ensuring they are allocated to the most relevant clients.
• Manage the client and carer relationship and ensure outstanding levels of service and care are delivered.

The Care Coordinator in Chelsea will need:
• Previous coordination experience within a homecare business.
• Sales experience within a homecare business/prior coordination role.
• Relevant Health and Social Care qualification.

The successful candidate will enjoy a range of benefits including:
• Base salary to £34,000
• Bonus
